Construction of a Time Line
Mention from conception to birth (as a felt sense of how it was or from what you’ve been told), from birth to the present day, all important emotional and physical traumas and shocks—rejection, heartbreaks, disappointments, diseases, family stress, divorce, work-related events, deaths in the family or of friends etc. that you remember experiencing. Mention the symptoms experienced at those moments, or those you can date back to those traumas. Particularly note any events from which you felt “never well since.” Use the repertory of shocks below as a guide.
    1. Pre-birth traumas: unwanted, unplanned-for, attempted abortion, being the wrong sex, toxicity due to mother’s smoking, drinking, drugs, or emotional rejection, disturbance due to parental upset, accident, or other external trauma.
    2. Birth traumas: prolonged labor, induced labor, anesthesia, use of forceps, Caesarian section, lack of oxygen, breech birth, a sense of being born to the wrong family, or of being separated from God.
    3. Natural childhood illnesses that were not properly dealt with (due to weakened resistance): measles, mumps, rubella, chickenpox.
    4. Vaccinations: any known reactions or long-term effects (especially from smallpox, pertussis and the MMR series, that remove the ability of the immune system to mature).
    5. Traumatic life events: emotional—fright, fear, rage loss, abuse, abandonment, adoption, etc. and physical—surgery, concussions, spinal injuries, molestation, environmental insults, accidents, hospitalizations, etc.
    6. Prolonged use of drugs, antibiotics, psychotropic drugs (antidepressants, antipsychotics, etc.), alcohol, cortisone or other steroids, gold salts, or stimulants (chronic drug uses and alcoholics are permanently in shock).
    7. Serious diseases: venereal disease, malaria, tuberculosis, mononucleosis, typhoid fever, cholera, hepatitis, etc.

Family History

Mention diseases of family members, ages and cause of death of father, mother, sisters, brothers and grandparents on both sides of your family.

Please list all conditions on physical, emotional and spiritual levels that are currently bothering you.
